Citizens Memorial Hospital
Citizens Memorial Hospital, along with the Citizens Memorial Health Care Foundation, operate a rural integrated healthcare system that serves an 8-county area in southwest Missouri with a population of more than 100,000. CMH officially opened the hospital Sept. 28, 1982.
The hospital offers acute care with an 86-bed facility, including medical/surgical, Intensive Care Unit, Telemetry, Birthing Unit (The Birth Place), Heart Center, Geriatric Psychiatry, and Level II STEMI Center. The Kerry and Snyda Douglas Medical Center offers a full array of outpatient services. CMH also provides emergency services with its Level III Trauma Center and ambulance services for four counties. CMH extends its service with 34 physician offices, 12 of which are certified Rural Health Clinics.
Citizens Memorial Health Care Foundation encompasses long-term care including six long-term care facilities with a total of 705 beds, one residential care center and five independent living communities for seniors; in-home care services, hospice, home medical equipment and health transit; and retail pharmacies in Bolivar and Osceola.
Mercy
Mercy Multi-Specialty Clinic and Urgent Care – Bolivar offers a full range of services from a compassionate team of caregivers for the entire family, including Mercy Kids. Mercy’s common goal is to provide high-quality care and an easier, more personal experience. Patient satisfaction is their top priority, with a strong focus on safety and innovation.
Mercy providers are connected by an advanced electronic health records system; therefore, the most up-to-date medical information is instantly available to onsite caregivers at each point of medical care.
OCHC – Ozarks Community Health Center
At Ozarks Community Health Center, patients can expect the highest level of care from their team of providers who have a wide range of specialties and are ready to help with all your needs. Our mission is to provide quality, compassionate, and professional healthcare to all with experienced providers, professional and friendly staff, and flexible hours. The Bolivar location offers dental services on a sliding fee scale to those self-pay patients who qualify. The office offers a sliding fee discount to those self-pay patients who qualify. Services at the Miles for Smiles include Dental Screenings/Oral Exams, Cleanings, X-Rays, Fillings, Crowns, Root Canals, Sealants, Dentures, and Extractions.
